Pune, July 27: Experts at the Army Institute of Cardio-Thoracic Sciences (AICTS) in Pune have achieved a remarkable feat in cardiac care by successfully performing a rare and complex minimally invasive heart procedure. This accomplishment signifies a major advancement in cardiac treatment within the Armed Forces Medical Services (AFMS). The procedure was conducted on a patient who had previously undergone heart valve replacement surgery and subsequently developed severe leakage in the heart's tricuspid valve on the right side.

The innovative procedure, known as bicaval transcatheter valve implantation (CAVI), involved the simultaneous placement of two prosthetic valves in the superior vena cava and inferior vena cava, the two principal veins that transport blood to the heart's right side. By employing this advanced catheter-based technique, the AICTS team effectively addressed the patient's condition without necessitating another open-heart surgery, thereby reducing both surgical risk and recovery time.

This achievement highlights the increasing capability of military medical facilities in India to provide cutting-edge structural heart interventions, a service offered at only a few highly specialized cardiac centers worldwide.

As reported by AICTS, the patient had earlier undergone heart valve replacement surgery but later suffered from severe tricuspid regurgitation. This ailment often leads to serious symptoms such as swelling in the legs and abdomen, liver congestion, fatigue, and breathlessness. For patients who have previously had open-heart surgery, undergoing another operation poses significantly higher risks due to scar tissue, past procedures, and related medical complications.

In light of these challenges, the multidisciplinary cardiac team at AICTS chose a minimally invasive catheter-based strategy. By utilizing advanced interventional techniques, they simultaneously implanted two prosthetic valves in the veins leading to the right atrium. These newly placed valves effectively prevent the backward flow of blood, enhancing circulation and eliminating the need for another major surgical procedure.

The patient experienced a successful recovery following the intervention, underscoring the effectiveness of this advanced treatment option for carefully selected high-risk patients.

Medical professionals describe bicaval transcatheter valve implantation as one of the most technically challenging procedures in the realm of structural heart disease. Unlike Transcatheter Aortic Valve Replacement (TAVR), which has gained widespread acceptance for treating aortic valve disease globally, CAVI remains an emerging therapy with limited experience worldwide.
