At 11:48 hours, an Indian Air Force Mi-17V5 helicopter took off from Sulur Air Base, Tamil Nadu, carrying General Bipin Rawat, his wife Madhulika Rawat, and twelve other defense personnel, including Teja. The aircraft was bound for the Defence Services Staff College at Wellington in the Nilgiris district. Around 12:20 hours, the helicopter crashed in a forested area near Coonoor, Tamil Nadu. Thirteen of the fourteen aboard, including Teja, perished. Group Captain Varun Singh, the sole survivor, was critically injured and hospitalized.
The loss of General Rawat and his companions, including Teja, deeply affected the nation.

Final Honours and Mortal Remains
On December 11, 2021, Teja's mortal remains were received with full military honors at Yelahanka Air Force Station, Bengaluru.
